Office Policies
Please complete all registration forms prior to your first office appointment.
If you are unable to keep your appointment and fail to give us at least 24 hours prior notice, you will be charged a cancellation fee of $25.
Requests for medical records will incur $50-$100 administrative fee.
Returned checks will incur a
fee of $25.
We will accept credit card payments via: Visa/MasterCard
If a referral is required by your insurance company, please secure the proper approvals prior to your appointment and call our office to confirm that the referral was received.
